About the Mechanic
The Double-Sided Cards / Transform mechanic is a mechanic based on the similar MTG concept and is first used by GA-16, the main character of my Yugioh Fanfic YGO SIXTEEN.
Basically, this mechanic replaces a card on the field with another entirely different card when conditions are fulfilled.

The Basics
A card that can Transform (i.e. have another side) will have a Sun symbol, called Obverse Side before the Card Name.
Then, one of the following will also appear on the card, based on the template used:
1: A field will be above the text box, showing what the card will transform into.
2: In the effect text, "Transform this card to "Card Name""will be bolded.
For Example:


A Transformable card will have (generally) 1 or more Reverse Side Counterparts, they'll have a Moon Symbol before the Card Name.
For Example:


Something important regarding Reverse Side cards:
1: They cannot be put into your Deck.
2: Similar to Tokens, if they leave the field, they'll disappear. In other words, a reversed side card can never be in your Hand, Deck, Extra Deck, GY or among your Banished cards.

Transforming
If an effect requires a card to be "Transformed", you take the corresponding Reverse Side card and use it to cover the card on the field. From now on, as long as this card is face-up, it's treated as the Reverse Side card and have everything on the original card overwritten by the Reverse Side card's effects. This persists until the Reverse Side card left the field by any way, in that case the original Obverse card is sent to wherever the card would go.

For Example: "VTuber Kokoro Yami" is tributed by your opponent to Special Summon "Volcanic Queen" to your side of the field. In that case, its Obverse Side, "VTuber Mirai Akari" is sent to the GY, then you place the "VTuber Kokoro Yami" card back to your deckbox - in YGOPRO, the card will simply disappear.

Detail Rulings on Transforming:
1: Transform a card by itself does not start a chain, nor is that a Special Summon in any way or form. However, do note that while Transforming a card does not start a chain, the effects that would lead to a card transformed could still start a chain, and could be negated, by extension negating the transform.
Example: An Obverse card you control would have the effect of "If another monster is Special Summoned to your side of the field: You can pay 1000 LP; Transform this card",  "Quickdraw Synchron" is Special Summoned by its own effect, then you activate this monster effect and paid 1000 LP, then your opponent activated "Divine Warth", negating your monster effect and destroyed it, in this case, you cannot transform the card since the effect that leads to its transformation has been negated.
2: Aside from transforming into different card names / types / statlines / effects, etc. You can transform a card type into any other card types, given that the card is in a position where it's allowed to, or is moved to a position where it's allowed to before the transformation.
Example: An Obverse Synchro Monster can be transformed into a Reverse Link Monster.
A Continous Spell can have the effect of "You can move this card to your Monster Zone as a Monster, then Transform this card" to transform it into a Monster Card.
2.1: However, if the old card type contains information that cannot be transferred to the new card type, it will be lost upon transformation.
Example: If an Xyz Monster with 2 Materials is transformed into a Synchro Monster, the Xyz Materials are sent to the GY.

Author's Suggestion on making cards of this Mechanic
1: Cards that can transform generally invokes "another side" of the card, in the example above, Kokoro Yami is Mirai Akari's alter-ego. Don't set two completely different types as transform just for the sake of giving a sudden attack on your opponent.
2: Usually, if your Reverse Side is stronger than the Obverse Side, it'll be a good idea to impose a cost on transformation.
3: Don't make effects that would transform a Reverse Side card back to Obverse Side. - Instead, do effects like "Send this card to the GY, and if you do, Special Summon "Original Card Name" from your GY" if you really want a card to transform back when needed.
